Testing Instructions:
- If not logged in, you should see a "Join Waitlist" button that prompts login
- If logged in, you should see a "Join Waitlist" button that opens a form
- If provider is accepting clients, the waitlist button won't show (it's replaced by booking buttons)
- Fill out the form and submit to join the waitlist
- After joining, you should see "On Waitlist (#position)" with a Leave button
